Ptosis: Sagging of the eyelid

Ptosis is the condition in which one or both eyelids sag and partially or completely cover the eye. This can affect vision and the appearance of the eyes.

Causes of Ptosis

Ptosis can be caused by several factors, including:

  • Aging: As we age, the muscle that lifts the eyelid (the levator muscle) can weaken, which can lead to ptosis.
  • Congenital ptosis: Some people are born with ptosis, often due to an abnormality in the development of the levator muscle.
  • Neurological conditions: Conditions such as myasthenia gravis, Horner's syndrome, or stroke can affect the nerves that supply the eyelid muscles, causing ptosis.
  • Trauma: Injuries to the eye or eyelid can lead to ptosis due to damage to the muscles or nerves.
  • Surgical complications: Ptosis can sometimes occur after eyelid surgery or other eye procedures.

Symptoms of Ptosis

The main symptoms of ptosis are:

  • Dropping of the eyelid on one or both eyes, narrowing the eye opening
  • Difficulty opening eyes fully
  • Loss of vision in the upper visual field, due to the eyelid partially covering the view
  • Fatigue or discomfort when trying to lift the eyelid

Treatment of Ptosis

The treatment of ptosis depends on the cause and severity of the condition:

  • Surgical correction: The most common treatment for ptosis is surgery to tighten or reposition the levator muscle to increase eyelid lift. In some cases, an artificial eyelid lift may be necessary.
  • Eyelid Spasms: Sometimes ptosis can be managed by using glasses or lenses that help alleviate the effect of the drooping eyelid.
  • Treatment of underlying conditions: If ptosis is caused by a neurological condition such as myasthenia gravis, treating that condition will also help improve the ptosis.

Complications

If left untreated, ptosis can lead to visual impairment, especially in the upper visual field. In severe cases, the constant effort to lift the eyelid can also lead to muscle fatigue and headaches. Early diagnosis and treatment are essential to prevent these complications.
